  4. Ankylosing spondylitis, rheumatoid arthritis, Grisel's sydnrome, Reiter's disease, pseudogout

  5. 3 millimeter (4 millimeters in rheumatoid arthritic patients) anything over 4 millimeters is considered a candidate for surgery
